California Healthy Kids Survey

The California Healthy Kids Survey (C.H.K.S.) is a modular, anonymous assessment recommended for students in grades 5, 7, 9, and11; some schools may also opt to survey students in grades 6, 8, 10, and 12.  It is focused on the five most important areas for guiding school and student improvement:


  • Student Connectedness, Learning Engagement/Motivation, and Attendance;
  • School Climate, Culture, and Conditions;
  • School Safety, including Violence Perpetration and Victimization/Bullying;
  • Physical and Mental well-being and Social-Emotional learning; and
  • Student Supports, including Resilience-promoting Developmental Factors (caring relationships, high expectations, and meaningful participation).


(C.H.K.S.) provides local schools and communities with data to identify the needs of youth (grades 5 and above) and to guide efforts to meet those needs.  More information can be found on the California Healthy Kids Survey (C.H.K.S.) website.  Below you will find links to the Core survey questions, along with the supplemental modules selected by NMUSD:
